Test Your Clinical Knowledge!



What is the most appropriate liver test to exclude liver cell failure in a routine work-up in elderly patients, especially patients with dementia?

  1. this is an easy question.. check for elevated liver enzymes ie ALT and AST

  2. Hmm.. not a very easy question afterall because liver enzymes indicate structural damage ie hepatocytes rupturing and releasing the enzymes.

    Correct Answer is aldbumin and prothrombin time. If the liver fails to function for any reason even in the absence of structural damage, it also ceases to carry out its functions in as a manufacturer of plasma proteins and coagulation factors. So we end up with a deficiency of these parameters.
